What causes mold in the dishwasher?

I have mold in my Kenmore dishwasher model number 665.15872000. The heating element has been replaced in the past. What could be causing this and how can I fix it?

Mold in the dishwasher is usually caused by too much foam, suds or improper rinsing of the dishes before washing. You can add ¼ cup of bleach after the unit has filled with water. Let it run a short cycle without dishes. This should kill the mold.
