Why does my chain keep coming off the chain bar?

I have an electric chainsaw, model no. 358.34141. I was cutting a tree and suddenly, the chain came off. I loosened the bar (unscrewed the two nuts), put the chain back on, ran it but it sounded very loose and then came off again. How can I put the chain back on and tighten the chain/adjust it?

Remove the 2 nuts that hold the chain cover on and remove the chain cover. Go to

and you can see the parts diagram of the saw. The chain cover is item 20. Clean the sawdust and oil from the cover and look on the end of the cover for the bar adjusting screw (item 44). This bolt has a pin on it that engages in a hole in the bar to move the bar when you adjust the screw.

Be sure you get the pin in the hole of the bar when you put the cover back on. Put the chain in place and tighten the screw (clockwise) to tighten the chain on the bar.

Tighten the chain until it keeps the teeth of the chain inside the bar but loose enough that the chain turns freely.

My Remington M30016AW has exactly the same problem and even with every thing lined up perfectly the drive sprocket seems to be very sloppy on the shaft it spins on leading me to believe there is a bushing missing - though I have been unable to find a bushing in the online manual parts list. Can you provide any help?
