Why does my chain keep popping off of my chainsaw?

It is a Craftsman 20" saw.

Brand new chain was just purchased. No sawdust or bar is not damaged. Chain is not loose.
I start it up and start cutting, it becomes loose and dangling within 5-7 cuts and then it pops off.

Things that can cause the chain to pop off the chainsaw are:

Chain adjusted too loose.

Teeth on chain bent or worn.

Groove in bar damaged or has sawdust build up.

Getting the bar and chain caught in a bind when a limb is being cut.
